Children's Mercy Health Network provides an array of health education resources to provide support for medication management, lifestyle concerns, and management of co-morbidities.
CMHN health education follows nationally recognized guidelines of care to teach self-management skills that can reduce emergency room visits, hospital admissions and improve quality of life.Health education is designed to help patients follow their prescribed treatment plan and develop a partnership with providers. The goal is to empower patients while supporting your plan of care.
Additionally, the educators are available to work side-by-side with clinic staff to reinforce skills and foster behavior changes for effective medical management.
